SUBJECT: Questionnaire from Food Research and Action Center

The State WIC office has been sent a questionnaire from the Food Research and Action Center (FRAC). We would appreciate assistance from the local agencies in our response to this questionnaire. Completion of the questionnaire by the LWP is not required but we are interested in your responses. The attached are the questions we would like responses from the LWPs about.
