TO: Local WIC Provider Administrators, WIC Coordinators and WIC Nutrition Coordinators
SUBJECT: Formula Approvals Week of May 23 - 27, 2005
SUBJECT: Breastfeeding Peer Counseling Program Federal Fiscal Year 2006

1. State WIC office staff who normally approves exempt formula requests will be out of the office May 23rd through May 27th attending the National WIC Conference in St. Louis.  During this week, the LWP Nutritionist or Registered Dietitian (RD) has permission to approve the formula for 1 month only.  They should follow up with a phone call or email message to D'Anne Ward or Kathy Mertzlufft by May 31.  LWPs who do not have a nutritionist or RD on staff may call the State WIC Office at 1-800-392-8209 for assistance.
2. If your agency is interested in participating in the Federal Fiscal Year 2006 BFPC program, please read the attached letter and complete the application form, and return it to Kathy Mertzlufft at mertzk@dhss.mo.gov or fax to 573-526-1470 by June 15, 2005.  If you have any questions, please contact her at 1-800-392-8209.
