TO: Local WIC Provider Administrators, WIC Coordinators and WIC Nutrition Coordinators
SUBJECT: Spring Cleaning for Computer Equipment
SUBJECT: Folic Acid Brochures and Baby Your Baby Keep Sake Books
SUBJECT: HemoCue wants to pick-up old analyzers.

  1. Spring is here and it’s a good time to get rid of old computer equipment. If you have any WIC desktop or laptop computers, computer monitors or printers that you do not need, now is the time to act. If the piece of equipment has a WIC tag and a DHSS tag (see picture of tags in attachment), the state WIC office is responsible for its removal. We will do the paper work and DHSS staff will remove it. If the piece of equipment has a WIC tag and does not have a DHSS tag, notify us and we will send you a letter that contains instructions on how to properly dispose of it.
    If you have equipment to remove (provide the WIC # of the piece of equipment) or have questions, contact Mark Davis at Mark.Davis@dhss.mo.gov or 1-800-392-8209.

  2. The attached is an order form to be used ONLY for requesting folic acid brochures and Baby Your Baby Keepsake Books. Please fill out the form as needed and fax it to Leslie Seifert, Bureau of Genetics and Healthy Childhood, at 573-751-6185 or e-mail to Leslie.Seifert@dhss.mo.gov. After Leslie reviews your request, she will send your request to the warehouse. If you have any questions, please contact Takako Tagami at 1-800-392-8209 or e-mail Takako.Tagami@dhss.mo.gov.
  3. It has been brought to the attention of the State WIC Office that some of the Local WIC Providers may have old microcuvette analyzers and expired microcuvettes. HemoCue will pick-up old analyzers and expired microcuvettes. In the event that you are an agency that has old analyzers or microcuvettes, please complete the attached memo and fax or mail it back to Crystal Collins. If you should have any questions, please contact Crystal Collins at 573-751-6204 or by e-mail at Crystal.Collins@dhss.mo.gov. After you fax your completed memo, WIC will identify for HemoCue which Local WIC Providers have items to be returned and schedule a time for HemoCue to collect the items.
