What Can You Do?

Traumatic Brain Injury

Department/Division Directors

  • Adopt and provide leadership with your staff regarding the concepts
  • Ensure the policies and procedures over which you have control encourage that a collaborative, person-centered approach is implemented as routine practice
  • Allocate resources for electronic capacity building toward a shared data vision for state agencies

Case Managers/Service Coordinator

  • Practice the concepts as you work with people, those with TBI and others
  • Reach out to include the right people to help assess the needs and plan with the clients you serve so that your efforts will be sustained over time

Hospital Discharge Planner

  • Contact the Department of Health and Senior Services Adult Head Injury Service to establish a relationship with the local TBI service coordinator
  • Ensure that your patients have a planned transition to community resources that can assist them to function at the highest level possible.

Service Provider

  • Adopt and provide leadership with your staff regarding the concepts
  • Ensure that policies and procedures are in place that allow direct care staff to participate in shared assessment, planning and information necessary for optimal delivery of quality services
