How long is the contract period?
The contract runs from May 1 through August 31, 2008.
When am I supposed to perform the inspection?
Initial inspections are to be performed within the first two week of the sites operation and are to be performed while food is being prepared or served.
When is a follow-up inspection necessary?
When there are critical violations, which cannot be corrected that day. Call Bryan Haugen at 417 895-6932 to obtain approval to conduct follow-up inspections. A follow up inspection without prior approval is not reimbursable.
Where do I send the completed inspection report?
Within 14 days of inspection, mail the inspection report to:

When should I contact CFNA?
When you are aware of a site that does not appear on the website or you are having trouble accessing the website contact Brandy Marchat 888-435-1464.
How will I know what sites to inspect?
By accessing the Summer Food Service web site at www.dhss.mo.gov/sfsp
select Related Links. Select SFSP Site List for Food Establishment Inspectors. Then enter your county and date range and select get report. The sites to be inspected will be listed. If you are aware of sites not listed, contact Brandy March, CFNA at 888-435-1464. (If you are under contract to inspect for another county you will access that counties information as above, you may also want to contact the county administrator or staff for additional information.)
How do I get reimbursed?
Submit one invoice for all inspections on a DH-38. They are available from the DHSS warehouse and are also available electronically. Do not use any other form, such as a Day Care form, to request payment. All invoices must be received by BERL by October 31, 2008.

What if I had network access last year for SFSP, do I need to re apply?
Network Access is no longer needed. Sites may be pulled up without access to the web-based system. If you feel you need further access than what is offered please contact Brandy March, CFNA at 888-435-1464.
How do I get a Missouri Department of Health and Senior Services issued EPHS number?
If you do not already have a state issued EPHS number you may get one by calling Alicia Baim at (573) 751-6090 or Kathy Sale at (573) 751-6095.
