Applications & Forms

Onsite Sewage

Applications and forms are in PDF and/or Microsoft Word format. Some applications are fillable, which can be typed into using your computer, and where applicable check boxes may be checked. When all fields have been completed, the printed form needs to be signed and dated before submitting to the Onsite Sewage Program or local authority as applicable. Keep a copy of each form for your records and mail the original completed form and any other necessary forms, information, and fee to the Onsite Sewage Program or to the local authority. If problems are encountered, contact us.

DHSS Construction Permit Applications

A permit must be obtained before starting onsite system construction or repair (except for a small number of residential properties that may be exempt from state and local permitting requirements).  When authority is based on a local ordinance, regulation can be more restrictive than the state minimum standards and the permitting processes and forms will be different.  The link below will assist you in finding the onsite sewage authority for your property, explain the DHSS Onsite Sewage Program permit/application processes, and link to downloadable DHSS Construction Permit Application forms.  Note that those forms can only be used in certain counties.

Site/Soil Evaluation Forms

Only Registered OWTS Onsite Soil Evaluators (OSE) or Percolation Testers, can conduct site/soil evaluations for OWTS design using the forms below or similar forms.
