Metabolic Formula Distribution

Medical eligibility for the Metabolic Formula Distribution Program is documented with a written medical diagnosis of one of the following conditions and other conditions approved by the Missouri Genetic Advisory Committee.

Amino Acid Disorders

Organic Acid Disorders

pdf file Methylmalonic acidemia (MMA)
pdf file Propionic acidemia (PROP)
pdf file 3-hydroxy-3-methylglutaryl-CoA lyase deficiency (HMG)
3-methylcrotonlly CoA carboxylase deficiency (3-MCC)

Financial eligibility guidelines are based on the U.S. Department of Health and Human Services Poverty Income Guidelines at, or below 185%, with an adjustment made by adding two (2) to the total number in the family. All third party payors must be utilized before the Department of Health and Senior Services (DHSS) will consider reimbursement. The DHSS is the payor of last resort. Applicants and program clients must apply for Medicaid benefits or medical assistance and complete the application process to obtain or maintain program eligibility.
