Metabolic Formula Program

The Metabolic Formula Program provides financial assistance to Missouri residents who meet the financial and medical eligibility criteria of the program. If you are not financially eligible for the program, service coordination is available to help you find providers of service for your condition.

Program Services:

• The Department of Health and Senior Services (DHSS) will provide financial assistance to program participants for the purchase of metabolic formula. Prescriptions for formula must be filled at a DHSS contracted provider.
  
• DHSS will provide financial assistance for an annual examination from one of the listed treatment centers.
