Missouri Healthcare-Associated Infection Reporting System (MHIRS)

MHIRS Registration Login
To access the MHIRS registration form, the healthcare facility must log on to the registration site. The login screen will prompt you to select your facility name and to enter your password. The password for the MHIRS registration has been mailed to each facility administrator.

Registration Form Instructions and Information
Once you have logged on to the MHIRS registration website, a screen will appear with your facility name and address information. You can then proceed to complete the registration form. Please note: An asterisk indicates a required data field.

• Contact Information:
• Primary Contact: this should be the person you want the DHSS to contact regarding infection data reporting issues and requirements.
• Secondary Contact: this is the person you want the DHSS to contact in the event the Primary Contact is unavailable.
• Intensive Care Units
• Definitions for ICU, NICU, central line, ventilator, central line days and ventilator days are embedded in the registration form for easy reference.
• Surgeries
• Descriptions for the surgery reporting categories, along with ICD-9-CM codes, are embedded in the registration form for easy reference.
