Can I fax paperwork in to your office?
Yes, you may fax your Maintenance reports, requests or applications. If faxing any paperwork, leave a number so that we may contact you if there is an error in transmittance. Faxing takes the place of mailing and is considered an original, so do not send hard copies via mail if you have faxed them. Ensure the legibility of each page before faxing.

What do I need to send in to you so that I can renew my Type 3 permit?
You should have performed at least 10 actual DWI subjects in the last year of that permit and submit the copy of the BA log for those tests with your application or if this has not been met than you will need to perform 5 self-administered breath test with 5 completed observational checklist or the 3rd page of the AIR form. You must also complete a 2 hour refresher course under the supervision of a valid Type 2 permit holder on the instrument for which you are submitting for renewal. Your permit application must have the attached copies of the BA log for the actual DWI’s performed or self-administered test and operational checklist. Retain copies of everything sent to us until you have received your permit from us. This will help relieve the aggravation associated with thing’s that can get lost in the mail, you will have a copy to fall back on if this occurs.

What do I send in with my new Type 3 permit applications?
If you are a Type 2 and you are submitting the applications of your newly trained Type 3’s, you must send us a Type 3 permit application and a copy of their final written exams for the course that they have just completed. If your training was performed by a training academy, you will need to contact the training academy and see what will be need for you to submit your application for permit, complete with all that is required. Or you can contact our office for more information to help in obtaining the necessary paperwork. Retain copies of everything sent to us until you have received your permit from us. This will help relieve the aggravation associated with thing’s that can get lost in the mail, you will have a copy to fall back on if this occurs.

What if my Type 3 permit has expired?
When a permit expires it can no longer be used, there is no grace period. An expired Type 3's permit that is still within the 30-day period from their expiration date will need to submit a completed application with either the 10 actual DWI’s if this is not met than you need to performed 5 self-administered breath test with 5 completed observational checklist or the 3rd page of the AIR form. If they are expired beyond the 30 days then a 2- hour refresher course will need to be performed by the Type 2 and during this time the Type 3 will perform their 5 self-tests. If it is beyond 6 months, but not quite a year, then the refresher class may be a little longer than 2 hours. Once they are expired by one year or more they have to have the Type 3-operator course all over again, which is 24 hours or more.

What do I need to submit to you if I want to conduct a Type 3 class?
You will need to submit a letter asking for approval to perform the Type 3 training to the Breath Alcohol Program. In the letter you should include: the location of the class, the type of class (24 hours for new a Type 3 permit or four hours for a new instrument), the dates of the class, instructor(s), approximate number of students and your course outline with the topics per hour and a master copy of your written exams with answer that will be given at the completion of the class.

New Type 3’s need a minimum of 24 hours, which includes the BA instrument. For a current Type 3 to change to another BA instrument is a minimum 4 hours with a written exam over the BA instrument.

What if have a current permit for one BA instrument and now need training on another, in addition to the current BA instrument?
This requires a minimum 4-hour course on the BA instrument complete with written exam. If you would like to have both BA instruments on one permit, renewal procedures for the current BA instrument must be met at the time the application request is made for new BA instrument. You should have performed at least 10 actual DWI subjects in the last year of that permit and submit the copy of the BA log for those tests with your application or if this has not been met, then you will need to perform five self-administered breath tests with five completed observational checklists or the 3rd page of the AIR form. Your permit application must have the attached copies of the BA log for the actual DWI’s performed or 5 self-administered tests and 5 operational checklists. A letter of approval will be sent to you, giving permission to conduct the training. A copy will be retained in the Breath Alcohol Program.

What do I need to send in to you so that I can renew my Type 2 permit?
To renew your Type 2 permit you will need to either fax or mail a Type 2 application completed per the instructions on the application:

Complete with your original training, list number of maintenance reports performed in the last year of the permit, the number of actual DWI’s performed in the last year of the permit (at least 10). If you haven’t performed at least 10 DWI’s in the last year of your permit you will need to perform five self-administered breath test and fill out 5 operational checklist or the 3rd page of the alcohol influence report page for each of the 5 breath test printouts. Your permit application must have the attached copies of the BA log for the actual DWI’s performed or self-administered test and operational checklist.Retain copies of everything sent to us, until you have received your permit from us. This will help relieve the aggravation associated with thing’s that can get lost in the mail, you will have a copy to fall back on if this occurs.

What is required to become a Type 2?
You must be 21 years of age and employed by the Law Enforcement Agency that owns the Breath Analyzer instrument. There may be two Type 2’s per instrument at the agency that owns the BA instrument, beyond these requirements it will be necessary to contact our office. If there is more than one Type 2 at your agency, it is expected that the Type 2 will have hands on proficiency with the instrument and the performance of the monthly maintenance report. The BA instrument responsibilities should be shared equally.

An approved training course is required before a permit may be issued from the Breath Alcohol Program. Prior approval from us is necessary to attend either of the training facilities.

There are two primary training facilities, the Missouri State Highway Patrol Academy located in Jefferson City, MO. and the Central Missouri State University, Missouri Safety Center located in Warrensburg, MO.

There have been occasions where we have provided Type II training to individuals here within our lab.

Beyond these requirements, it will be necessary to contact our office.

What paperwork do I send in for my monthly maintenance report?
Remember that you always keep the originals.

For the Datamaster: Copies of the evidential printouts of the: Diagnostic Check, Calibration Check, and the R.F.I., the Certificate of Analysis and a copy of your current Type 2 Permit should be sent to us within 15 days of the performance of the maintenance report(s). Remember that you always keep the originals.

For the Intoxilyzer 5000: Copies of the evidential printouts of the: Diagnostic Check, Print Test, Calibration Check, and R.F.I., the Certificate of Analysis and a copy of your current Type 2 Permit should be sent to us within 15 days of the performance of the maintenance report(s). Remember that you always keep the originals.

The same is true if a 0.04% standard maintenance report is performed.
Only one Type 2 permit holder shall perform and sign the Maintenance Report.
Ensure the legibility of each page.

What if I am having instrument problems whom do I contact?
If you happen to experience any problems with your BA instrument, you should contact us first at 573-840-9140. We need to be aware of any problems and what type of problems may be occurring with the instruments.

We are Manufacture trained on the BA instruments, and stay current with yearly updates provided by the manufacturer. We do know how to troubleshoot problems with the instruments and perform repairs. Most problems can usually be resolved over the phone, if not, we can direct you to the appropriate place.

How many breath analyzer instruments do we have in use within our state?
We have 473 breath analyzers (BA instruments) in use within our state:
385 Datamasters: 125 used by the Missouri State Highway Patrol, 260 used by police departments, sheriff departments, and Missouri State Water Patrol. Representing 224 non-highway patrol agencies where these datamaster’s are located.

Intoxilyzer 5000, 84 located within 56 police departments and one sheriff department.

Alco-Sensor IV with printer* six, located in two different Police departments.
In addition to the BA instruments in use within our state there are 19 Breath Alcohol Testing (BAT) Vans. These are specially designed and equipped vans used mostly for Sobriety Check points.
