Requesting a Background Screening

Background screenings may be requested by Internet, phone, fax, or mail on registered individuals.
Internet
Access to requesting background screening information using the Internet is granted to approved providers licensed by or contracting with the Department of Health and Senior Services (DHSS) who are using the information for employment purposes as defined in Section 210.921.1, RSMO. Approved providers must have an authorized Memorandum of Agreement on file with DHSS and each individual accessing the internet application must obtain a UserID and establish a password. Phone
Requests for background screenings may be made by phone using the toll-free access line, 1-866-422-6872, between 7:00 a.m. and 6:00 p.m., Monday through Friday. The caller must provide their name and address as well as the registrant’s name and social security number. The caller will receive results over the phone along with a confirmation number. Both the caller and registrant will receive written confirmation of the results by mail. If matching records were found, the requestor must have their state agency licensing number to receive specific information over the phone.
Fax
Requests for background screenings may be made by submitting the Employer Background Screening Request form by fax to 573-522-6981. The background screening results will be returned to the requestor and registrant by mail.
Mail
Requests for background screenings may be made by submitting the Employer Background Screening Request form by mail to the Family Care Safety Registry, P.O. Box 570, Jefferson City, MO 65102. The background screening results will be returned to the requestor and registrant by mail.
