Adult Day Health Care (ADHC) Provider Information

Adult Day Health Care Services (ADHC) are organized programs consisting of therapeutic, rehabilitative and social activities provided outside the home, for a period of less than twenty-four (24) hours, to persons with functional impairments of at least a nursing facility level of care. 
ADHC is funded through MO HealthNet with the Department of Social Services, MO HealthNet Division (MHD) and Social Services Block Grant (SSBG) with the Department of Health and Senior Services.

SSBG funds are used only during periods of MO HealthNet ineligibility.
